The physicians and staff at Southeastern Neurosurgical and Spine Institute look forward to meeting you. Our office hours are 9:00 a.m. until 5:00 p.m. Monday through Friday. At the time your first appointment is scheduled, our office will send you Patient Information and Patient Health History forms along with a map to our office. These forms are available on this website for download if time does not permit delivery prior to your appointment date. It is extremely important for you to complete these forms before your appointment, since they are required for check in. You may also fax the completed documents prior to your visit to (864) 454-4605. This will allow our staff to more promptly serve you and may alleviate any possible delay in seeing a physician. Please remember to bring a copy of past medical records along with any x-rays, MRI’s, CT scans, or myelograms you may have relating to your problems. Providing this information will prevent the need for repeating studies unnecessarily. In addition your physician will need complete information to aid in your diagnosis and treatment. If we do not have this information, your appointment may be cancelled and rescheduled for a later date. Please be aware that our doctors are sometimes called to an emergency on short notice and appointments may need to be rescheduled. If this should happen, be assured that you will be given the next available appointment. We ask for your understanding and please know that our doctors will give you the same priority treatment if the need arises. It is always a good idea to contact our switchboard (Dial 0) prior to leaving for your appointment to verify that the doctor has not been called out to an emergency. We ask that you bring your insurance cards and information
regarding your insurance companies’ requirements for second opinions
and pre-authorization of hospital services to each of your office visits.
Southeastern Neurosurgical and Spine Institute participates with most
insurance plans, however, you should contact your insurance provider to
determine if our services are covered. Patients are required to pay insurance
deductibles or co-payments when they check-in. We realize payment issues
can be confusing. Please contact our Patient Account Representatives at
864-454-4600 if you have any
questions.
